Introduction
This page hosts links to various forms used (initiated) by current UIC CLJ graduate students. They are controlled by either the CLJ Department or the Graduate College. All forms (regardless of who controls them) should first be forwarded to the CLJ Graduate Studies Offices. Independent Study Request
Please use this form to submit a formal request to register for an Independent Study within the CLJ department. This form is to ensure Independent Studies are being taken as they were intended. It must be approved and signed by both your advisor and DGS. Please see below course catalog description for clarity. Email the completed form to IS Form
CLJ 596. Independent Study or Research. 2-8 hours.
Research undertaken for this course may not duplicate that being done for CLJ 598. Supervised projects, which may consist of extensive readings in criminology, law, and justice, research on special problems not included in the regular course offering. Course Information: May be repeated. Students may register in more than one section per term. Prerequisite(s): Consent of instructor and approval of the director of graduate studies.
Spring 26 Course Registration
Please use this form to submit approval requests for CLJ courses you would like to register for during the Spring 2026 semester. Once completed, email to Spring.

CLJ BOT Waiver Request & TA Application
BOT Waiver Application
This form is for current CLJ students who would like a part-time/full-time tuition waiver from the department. Please complete and email the form to BOT Waiver
CLJ Teaching Application
This form is for current CLJ students who would like to apply for a teaching (TA/instructor) position with the department. Hiring decisions are determined by the CLJ Hiring Committee, which consists of current CLJ Faculty who have volunteered their time. Please be sure to submit your application by the April 1, 2026, deadline. Applications will not be accepted after this date.

Employment decisions regarding TA positions are based on the following factors:
- The number and type of undergraduate courses offered by the department.
- The amount of funding authorized by the college/university.
The Department will provide prospective TAs with an application no less than 45 days before offer letters are due. On the form, students can indicate their preference for type of appointment: 25% TA, 50% TA, or 50% TA instructor. Applications for 50% TA instructors require additional materials. Students must return the form by the articulated deadline.
Regarding selection, the following factors are considered by the CLJ hiring committee:
- Student’s academic performance and progress toward degree completion
- Schedule availability (students will not be given assignments that conflict with their course schedule)
- Previous job performance as a TA
- Prior experience related to a specific course assignment
- Prior experiences with academic community
- Prior teaching experience outside the university
- Number of years funded by the department (those with fewer years are prioritized)
- Doctoral student

Graduate Student Petition (Online)
Graduate Student Petition Use Scenarios:
- Extension of Time to Degree
- Extension of Doctorate Preliminary Exam
- Extension of Academic Probation
- Zero Hours (specify whether Option A or B)
- Defense Less Than 1 Year From Preliminary Exam
Petition to Register After the 10th (5th in Summer) Day
If you missed the registration period without registering for any courses and you need to be registered, you must obtain a Petition to. Register After the Deadline from the Office of Registration and Records. Complete that petition along with the Graduate Student Petition and Graduate College Registration Revision Form, obtain signatures as needed and submit all to the Graduate College for review. If approved, the Graduate College will send all forms to the Office of Registration and Records for their final review. Approval is not guaranteed, and if approved, a late registration charge will be added to your bill.

- Adding a Course to Your Existing Schedule After the Close of Registration
- Corrections to Sections of the Same Course or to Hours for Variable-Credit Courses
- Dropping Courses After the Deadline
